Job Summary

The HMIS/Medical Records Officer is responsible for the management, organization, and maintenance of medical records and health information systems (HMIS) within the facility. This role involves collecting, analyzing, and securing health data to support efficient and effective patient care delivery, ensure accurate reporting, and enhance quality improvement initiatives.

Key Responsibilities

  1. Data Collection and Entry
    • Collect, compile, and enter patient data accurately into the health information system.
    • Ensure all data is complete and up to date in accordance with regulatory standards.
    • Coordinate with clinical staff to verify and update patient information.
  2. Data Management and Analysis
    • Maintain patient health records in a secure and organized manner.
    • Conduct routine audits of data for accuracy, consistency, and completeness.
    • Analyze health information trends and prepare reports for decision-makers.
  3. Record Keeping and Storage
    • Implement and follow guidelines for filing, retrieval, and archiving of medical records.
    • Ensure the confidentiality and privacy of all health information in accordance with data protection laws and policies.
    • Safeguard physical and electronic records to prevent unauthorized access, theft, or damage.
  4. Reporting and Documentation
    • Prepare daily, monthly, and quarterly HMIS reports as required by the organization and health authorities.
    • Support the preparation of patient discharge summaries and other clinical documentation.
    • Contribute to quality improvement projects by providing relevant health data and insights.
  5. System Support and Troubleshooting
    • Assist in maintaining and troubleshooting electronic health record (EHR) systems.
    • Provide training to clinical and administrative staff on the use of HMIS/EHR systems as necessary.
    • Liaise with IT support teams to resolve issues related to system downtime or errors.
  6. Compliance and Policy Adherence
    • Ensure compliance with health information standards, laws, and facility policies regarding data privacy and security.
    • Stay updated on changes in HMIS and health data management practices to ensure ongoing compliance.
    • Participate in policy updates and help enforce adherence across departments.

Qualifications

  • Education: Bachelor’s degree in health information management, Medical Records, Public Health, or a related field.
  • Experience: At least 3 years of experience in health information systems, medical records, or data management.
  • Skills:
    • Strong organizational and analytical skills.
    • Proficiency in HMIS/EHR systems and Microsoft Office Suite.
    • Excellent written and verbal communication skills.
    • Understanding of confidentiality requirements and data protection laws.

Job Description:

Position Overview

The Drug Demand Reduction Technical Project Officer is responsible for supporting RHDO's drug demand reduction initiatives, working directly within community programs aimed at reducing substance abuse. This role involves planning, implementing, monitoring, and evaluating programs that address the root causes of substance misuse, with a focus on prevention, education, treatment, and rehabilitation. The Project Officer will work closely with local communities, healthcare providers, and stakeholders to ensure program success and positive community impact.

Key Responsibilities

  1. Program Planning and Development
    • Assist in the development of demand reduction strategies and intervention programs.
    • Conduct community assessments to identify areas and populations at high risk of substance misuse.
    • Design, adapt, and implement educational materials and resources for awareness and prevention.
  2. Project Implementation
    • Coordinate and manage day-to-day project activities, ensuring compliance with RHDO standards.
    • Facilitate workshops, training, and awareness sessions with community leaders, educators, and health professionals.
    • Work with healthcare facilities to improve treatment and rehabilitation services for individuals struggling with substance abuse.
  3. Monitoring and Evaluation
    • Develop metrics and tools for evaluating program effectiveness, collecting data on key indicators.
    • Analyze data to measure program impact, identifying areas for improvement and reporting on progress.
    • Ensure accurate documentation and timely reporting to donors, stakeholders, and RHDO management.
  4. Stakeholder and Community Engagement
    • Foster relationships with local community leaders, healthcare providers, and NGOs for collaborative efforts.
    • Act as an RHDO representative in meetings, conferences, and workshops related to drug demand reduction.
    • Engage families and community members in support networks for individuals undergoing treatment.
  5. Capacity Building
    • Conduct training sessions for local staff and community volunteers on prevention, harm reduction, and rehabilitation practices.
    • Provide technical assistance and guidance to healthcare and social services personnel involved in drug demand reduction.
  6. Advocacy and Policy Support
    • Assist in advocating for drug demand reduction policies at local and national levels.
    • Support RHDO in preparing reports and policy recommendations to improve public health approaches to substance misuse.

Job Requirements:

Qualifications and Requirements

  • Medical Doctor, Degree in Public Health, is preferred.
  • Minimum of 3 years of experience in public health, particularly in drug demand reduction or community health projects.
  • Demonstrated understanding of substance misuse issues in Afghanistan and experience working with high-risk populations.
  • Strong project management skills with experience in implementing, monitoring, and evaluating programs.
  • Proficiency in local languages (Dari and Pashto) and strong communication skills in English.
  • Ability to travel frequently to project sites and engage with diverse communities.
